1. Complete the TOURNAMENT ENTRY FORM,
     Write check to "Joe Millon"
     Mail check and entry form to:

The deadline is June 1.
Bids accepted with $50 late fee until June 12.
No refunds for teams that drop after June 12.

2. Do the PAPERWORK:

a) Complete a roster (online is strongly encouraged): www.upa.org/outreach/rosteringguide

b) Collect UPA waivers of liability for every player: www.upa.org/membership/forms

c) Update each player's UPA membership online or at the tournament, or include in your team fees the one-time event fee of $10 per each non-UPA player: www.upa.org/members/login.php

d) For all players under age 18, complete medical authorization forms: www.upa.org/membership/forms

e) For a team with players under age 18, complete a chaperone form: www.upa.org/membership/forms
